Self-Enrollment for HealthELife

Self-enrollment is available to any MIT Health patient aged 16 and older who does not currently have a HealthELife account. To begin the process, fill out the form below. To verify your account, choose "Email address," and enter the email address at which you receive communications from MIT Health.

If you manage the health of an MIT Health patient, ask that patient’s healthcare provider for a personal invitation to HealthELife during the next visit. You cannot use this self-enrollment page to open an account for another person, even if you are that patient’s guardian or caregiver.
